Will it not be when someone else has done the majority of damage to someone and then you come along and do a tiny bit of damage which kills them? |

This used to happen when you stand on a enemy tp-exit and someone spawn on it. That will make you die. But Baal is a Demoman??? |

it's the same symbol with the skull by a tp-kill, but the mysterious thing is the "finished off" xD
