OR, it might be the explosion that happens when an engineer tells gis building to self destruct, as I've had a mysterious 'kill' message come up (just a skull and crossbone with no texr, just my name on the left and someone from the other team on the right) when I destroy one of my own buildings without knowing there's a spy camping next to it

I have been telefragged before, unless its the engi teleporting with his pda out and kills you, I doubt he would get the kill listed as a pda kill. It might just be listed as an environmental kill though.
